I understand that animal(s) in my care for this trial visit:

Must be provided food and water at all times;

Must be provided a safe, well-vented area;

Must not be left alone with other animals unfamiliar to The HSRC animal(s);

Must not be removed from the approved home and transported to any other site;

Must not be left without human companionship for more than 8 hours per day;

Cats must be kept exclusively indoors and provided a clean litter pan;

Dogs must be kept indoors with frequent walks on a leash with well-fitting collar or harness (certain dogs as identified by The HSRC may be kept in a substantial fenced area if approved by The HSRC).

I understand that if the animal(s) becomes lost or stolen, ill or injured or presents a behavioral problem I must contact the representative listed below and The HSRC at the number and email address at the top of this page.

At the end of the trial visit I will contact the representative listed below and/or The HSRC at the provided contacts to pick up the animal(s) or to discuss completing the adoption process.
